Head to HealthMark’s Request Manager at requestmanager.healthmark-group.com and follow these easy steps:

  • Enter your email address.
  • Check your inbox for a secure login link from HealthMark — no username or password needed!
  • Click “Submit Request” and follow the prompts.
  • We’ll email you as soon as the records are ready to download.

Individuals authorized to act on a patient’s behalf may be required to provide legal documentation demonstrating their authority, such as guardianship documents, a court order, power of attorney, adoption records, estate administration documents, or similar legal paperwork.

Nope — one less thing to remember! HealthMark’s Request Manager uses email verification and secure links to get you the records quickly and safely, without another password to juggle.

Most requests are processed within 8 business hours. In most cases, records are delivered right to your inbox within one to two business days.
