Referral Contest Official Rules

NO PURCHASE NECESSARY to enter or win. You do not have to be a patient or schedule an appointment in order to enter the contest. This contest is open to every person over the age of 18. Odds of winning will depend on the number of eligible entries received. One entry per person is allowed.

How to enter:
For a chance to win the contest you must submit your entry by referring a family or friend to one of our Hero Practice Services practice locations. If you are selected as a winner and meet the eligibility requirements set forth herein, we will reach out to you directly via email.

Winners/Prize:
One (1) prize will be awarded for this contest. Approximate retail value of prize is $200.00

Publicity release:
By entering, each entrant acknowledges that his/her entry in the contest constitutes the entrant’s consent to use, publish, reproduce and for all purposes, including publicity, promotion and advertising, in any media (including, without limitation, the Internet, television of offline promotions), each winner’s name, voice, likeness, photograph, opinions, and or hometown and state, and any portion thereof, each extending throughout the universe and in perpetuity without further compensation, credit, or right of review or approval, except where prohibited by law.

Disclaimer:
If this contest is not capable of being held as planned, including, but not limited to tampering, unauthorized intervention, fraud, technical failure or other cause beyond control which corrupts or threatens to corrupt or adversely affects the administration, security, fairness, integrity or proper conduct of the drawing, Hero Practice Services reserves the right, in its sole discretion, to cancel, suspend and/or modify this drawing.

Limitation on Liability:
Winner, by acceptance of the prize, (a) agrees to the Official Rules and decisions of Hero Practice Services and (b) releases Hero Practice Services and its officers, directors, employees, and agents (collectively, the released Parties”) from any and all liability, claims or actions of any kind whatsoever for injuries, damages or losses to persons and property which may be sustained in connection with the receipt, ownership or use of the prize or while traveling to and from a location to obtain or use the prize. The Released Parties do not make any warranty, representation, or guarantee, express or implied, in fact or in law, relative to the use of the prize including, without limitation, quality, merchantability, safety, or fitness for a particular purpose. In consideration for his/her participation in the contest, each entrant agrees to hold harmless and indemnify the Released Parties from any and all claims, demands, or causes of action of any nature or kind whatsoever, whether presently known or unknown, foreseen or unforeseen, that arise out of entrant’s participation in the drawing, including attorney’s fees.
